Employees were performing roofing operations at an elevated height of approximately 20 feet on a pitched roof without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a worksite located at 11025 Jackson, Kansas City, Missouri. Employees were exposed to a fall hazard by using a ladder which extended approximately 2 feet above the landing surface of the roof to which was being accessed.
