29 CFR  1910.269(a)(2)(ii)(E): Each qualified employee was not trained and competent in the recognition of electrical hazards to which the employee may be exposed and the skills and techniques necessary to control or avoid these hazards:  11 Riley Road, Northborough, MA, on or about June 14, 2023:  An employee was exposed to electrical shock and burn hazards posed by an energized 13 kV overhead line while performing tree trimming activities in an aerial lift and was not trained or competent in the recognition of electrical hazards or the skills and techniques necessary to avoid these hazards.
