29 CFR  1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a):  Location: Fabrication Shop  On or about June 14, 2023, and August 1, 2023, employees were exposed to struck by hazards from flying particles when a Makita 9557ND angle grinder and 4.5" x 1/4" PFERD grinding wheel where used to grind metal welds without the use of safety guards.

29 CFR  1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:  Location: Fabrication Shop  On or about June 14, 2023, and August 1, 2023, employees were not protected from electrical shock and burns when damaged extension cords were not removed from service.
