This was most recently documented on June 16, 2023 at 2867 Camden Avenue, Omaha, Nebraska. The employer failed to ensure that multiple employees were protected from fall hazards when performing roofing construction work at a height greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  The employer was failing to protect employees from falls. This was most recently documented on June 16, 2023 at 2867 Camden Avenue, Omaha, Nebraska. The employer failed to ensure employees were protected from fall hazards when utilizing portable ladders that were used for access to the house roof and did not extend at least 3 feet above eave of the roof.
