This was most recently documented on June 16, 2023 at 2867 Camden Avenue, Omaha, Nebraska. The employer failed to ensure employees were protected from fall hazards when utilizing portable ladders that were used for access to the house roof and did not extend at least 3 feet above eave of the roof.
