(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling.  a.  On or about June 20, 2023, employees were exposed to a fall hazard of approximately 6' from damaged gates around solidification pits.

29 CFR  1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a.  On or about June 20, 2023, employees operated powered industrial vehicles, including forklifts, without certification.  Alpha Industrial Waste Services Corporation was previously cited for a violation of this Occupational Safety and Health standard, 1910.178(l)(6), which was contained in OSHA inspection number 1550601, citation number 2, item number 3 and was affirmed as a final order on September 1, 2022, with respect to a workplace located at 745 North 3rd Street, Cambridge, OH 43725.
