29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about June 21, 2023, a roofer was decking the roof of a new apartment building under construction without fall protection, exposing the roofer to a fall hazard of approximately 30 feet to the ground below.

29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about June 21, 2023, a roofer was working on the third story roof of a new residential apartment building under construction using a pneumatic nail gun and circular saw without eye protection, exposing the roofer to the hazard of being struck by flying particles and projectiles.
