29 CFR  1926.404(b)(1)(i):Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:   Worksite located at 4108 Davidson Road, Kansas City, Missouri- Employees are exposed to shock or electrocution hazards in that an electrical cord being used by the employee to power a radial saw was not plugged into a ground fault circuit interrupter or an assured grounding conductor program.

29 CFR  1926.404(f)(6):The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  Worksite located at 4108 NE Davidson Road, Kansas City, Missouri- Employees were exposed to a shock or electrocution hazards in that the electrical cord being used to power the radial saw had its' grounding pin removed, thus the path to ground was not permanent and continuous.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Worksite located 4123 NE Davidson Road, Kansas City, Missouri- Employees were exposed to fall hazards while working on 2nd story decks and were not protected from falling by the use of any means of fall protection.
