1915.116(b): Loads were not safely rigged before being hoisted. 375 Bath Road, Brunswick, ME - Employees were exposed to a crushing hazard when an Anver M150S standard single pad, self-powered vacuum lifter was used to lift a piece of steel plate that was over the load-rating of the vacuum attachment and was not centered on the load. The attachment was rated at 1500 pounds, as displayed on the front of the attachment, and was used to lift a plate that weighed 1690 pounds.
