Crossbraces were used as a means of access. a) On or about June 22, 2023, at 11052 E Colonial Avenue, Orlando, FL 32817: employees were exposed to a 16 feet 8 inch fall hazard, in that, employees were climbing the scaffold frame and were not provided a safe means of access to upper levels of the scaffold.

On or about June 22, 2023, at 11052 E Colonial Avenue, Orlando, FL 32817, employees working on a two tier tubular welded frame scaffold were not trained to recognize the hazards associated with the scaffold and procedures to minimize those hazards in that they were exposed to a 16 feet 8 inch fall hazard when climbing the frame to access the scaffold and working from a platform that was not fully planked.
