29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)  Jobsite:  On or about June 24,2023, employees were working at replacing an asphalt shingle roof and none of them were protected from falling to the ground or pavement.  The fall hazard ranged from approximately 11 feet to 18 feet.   The pitch of the roof was about 7 in 12.

29 CFR  1926.503(c):  The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:  (a)  Jobsite:  On or about 6/24/2023, employees need to be retrained on how and when to properly use personal fall protection when working from a roof.  None of the employees were tied off while working to replace an asphalt shingle roof.  The fall hazard was from about 11 feet to about 18 feet.
