29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  a) Multistory Commercial Building; Floor #20: On or about 08/16/2023, and at times prior, the employer exposed employees to fall hazards at heights greater than 25-feet to the ground level below, in that the employer did not provide the employee with an independent anchor point for the POLYDAC Model: 71-50 lifeline rope, that supports at least 5000 pounds while crossing the upper crane tie-in to access the crane stairwell.

29 CFR  1926.851(c): In a multistory building, when a stairwell is being used, it shall be properly illuminated by either natural or artificial means, and completely and substantially covered over at a point not less than two floors below the floor on which work is being performed, and access to the floor where the work is in progress shall be through a properly lighted, protected, and separate passageway.  a) Multi-story Commercial Building; Stairwell: On or about 06/27/2023, and at times prior, the employer exposed employees to trip hazards, in that the employer did not provide employees with an alternate means of illumination when traveling up and down the stairwell from floor(s) #7 to #22 for employees to remove HVAC material, remove outer panels, demolish/cut concrete, operate powered machinery, and remove glass from the outer wall areas of the building.
