29 CFR  1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  On or about June 27, 2023, and times prior to, an active outlet that is located next to the exit door where employees walk by on a daily basis had a missing cover to an electrical outlet.  This exposed employees to electrical hazards.

29 CFR  1910.101(b): "Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6.  On or about, June 27, 2023, and at times prior to, five cylinders of Helium were stored, unsecured, in the backroom storage area against boxes and a wire rack.
