29 CFR  1926.501(b)(1):Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, or safety net systems.  On or about June 27, 2023, and at times prior thereto, two employees engaged in commercial roofing operations at about 25 feet high without fall protection. This exposed employees to fall hazards.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 27, 2023, and at times prior thereto, employees conducting roofing operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls of about 25 ft.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 27, 2023, and at times prior thereto, employees used a ladder for access and egress purposes at a height of about 25 feet.  The ladder did not extend as least three feet above the landing.  This exposed employees to fall hazards.
