29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a.  GT Technologies at a worksite located at 99 North Fearing Boulevard, Toledo, Ohio 43607:  On or about June 26, 2023,  the employer did not ensure energy control procedures for the "Minster 400 ton hydraulic Press # 211" were adequately developed, documented and utilized to ensure control of all hazardous energy sources.  An employee performing finger adjustments on press # 211 in energized state,  was exposed to caught-in and crushing hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a.  GT Technologies at a worksite located at 99 North Fearing Boulevard, Toledo, Ohio 43607:  On or about June 26, 2023,  the employer did not ensure periodic audits of the energy control procedure for the Minster 400 ton hydraulic Press # 211 were completed at least annually.  Employees tasked with  servicing and maintenance activities on the "Minster 400 ton Hydraulic Press - #211," without the benefit of energy control procedures that have been evaluated for effectiveness and/or deficiencies, were exposed to caught-in  hazards.

29 CFR  1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:   a.  GT Technologies at a worksite located at 99 North Fearing Boulevard, Toledo, Ohio 43607:  On or about June 26, 2023, The employer did not ensure that employees tasked with service and maintenance activities were adequately trained in the application of energy control in which isolate and control hazardous energy sources during both maintenance and servicing tasks on the Minster #211 Press. Employees performing these tasks without the benefit of adequate training were exposed to crushed-by hazards.
