29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 29, 2023 employees were exposed to a fall hazard of approximately 12 feet while performing roofing activities on a residential home located at 1946 Wynwood Drive, Rocky River, OH . The employer did not ensure that employees performing roofing activities on a steep slope roof were protected from fall hazards. Employees were exposed to fall hazards of 8 -12 feet to the ground below while performing roof tear off and shingle replacement.   Spartan Weathershield Co. was previously cited for a violation of this occupational safety and health standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1312814, citation number 1_, item number 2 and was affirmed as a final order on July 6, 2018, with respect to a workplace located at 8455 Cherry Hill, Broadview Heights, Ohio 44147.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about June 29, 2023 employees were exposed to a fall hazard of approximately 8-12 feet while performing roofing activities on a residential home located at1946 Wynwood Drive, Rocky River, OH . Employees were using an extension ladder to access the roof and the employer did not ensure they extended at least 3 feet above the upper landing surface.  Spartan Weathershield Co. was previously cited for a violation of this occupational safety and health standard 29 CFR  1926.1053(b)(1), which was contained in OSHA inspection number 1312814, citation number 1_, item number 3 and was affirmed as a final order on July 6, 2018, with respect to a workplace located at 8455 Cherry Hill, Broadview Heights, Ohio 44147.
