29 CFR  1910.28(b)(11)(i): The employer did not ensure that each employee exposed to an unprotected side or edge of a stairway landing that is 4 feet (1.2 m) or more above a lower level is protected by a guardrail or stair rail system.   (a) Northside Production - On or about June 29, 2023, employees were exposed to a fall hazard due to the lack of a mid-rail for two sets of stairs leading to the dock, exposing employees to fall hazard.
