Jobsite - On or about June 29, 2023, employees utilizing a JLG 1200SJP aerial lift were not protected from an ejection hazard where personal fall restraint equipment was not worn and connected to the basket of the lift.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Jobsite - On or about June 29, 2023, each employee working on the roof of the building around the base of the steeple was not protected from a fall of approximately 25 feet where a fall protection system was not utilized.
