29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i)(A-C):  a) On or about July 5, 2023, while doing periodic inspections, sampling, and maintaining, equipment and product on the the mezzanine level of the heat exchangers, "E-4003 NaOH dilution to hypo" sample point and the mezzanine level of the Electrolyzer Cell Island, employees were exposed to fall hazards up to approximately 9 feet; the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge approximately 9 feet above the lower level was protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i)(A) Guardrail systems; 29 CFR 1910.28(b)(1)(i)(B) Safety net systems; 29 CFR 1910.28(b)(1)(i)(C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.

29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface.  a) On or about July 5, 2023, while walking up the stairs to do periodic inspections, sampling, and maintaining, equipment and product on the the mezzanine level of the heat exchangers, employees were exposed to fall hazards up to approximately 6 feet due to a midrail positioned at 16-inches ; The employer did not ensure that guardrails met the requirement by installing the midrail at 21-inches, halfway between the 42-inch top edge of the guardrails and the walking-working surface.

29 CFR  1910.305(h)(6): Fittings. Connectors used to connect lengths of cable in a run shall be of a type that lock firmly together. Provisions shall be made to prevent opening or closing these connectors while energized. Strain relief shall be provided at connections and terminations.  a) On or about July 5, 2023, maintenance employees rebuilding pumps and motors; and replacing failed pumps are exposed to a 240-volt electrical shock from the fitting connecting the cable  to the grey motor under the heat exchangers mezzanine; the employer did not ensure that the connectors used to connect lengths of cable in the run locked firmly together or provisions were made to prevent opening or closing the connector while energized.

29 CFR  1910.1200(f)(6)(ii):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a) On or about July 5, 2023, the employer did not ensure the Depleted Brine container along the north wall was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof,  which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.
