29 CFR  1910.252(a)(3)(i):Used containers. No welding, cutting, or other hot work shall be performed on used drums, barrels, tanks or other containers until they have been cleaned so thoroughly as to make absolutely certain that there are no flammable materials present or any substances such as greases, tars, acids, or other materials which when subjected to heat, might produce flammable or toxic vapors. Any pipe lines or connections to the drum or vessel shall be disconnected or blanked.  a) On or about 6/5/2023, employer did not ensure that employees operating a cutting torch on a used closed barrel was cleaned of residual flammable chemicals (compounds detected include benzene and toluene) prior to cutting exposing the employees to explosion hazards.

Employee training shall include at least:  (iii):The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used  a) On or about 07/05/2023, the employer did not ensure that employees were following the specific procedures to protect themselves from exposure to flammable chemicals (Toluene and Benzene) when employees attempted to open sealed used barrels with a torch cutter or a reciprocal saw.
