29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  Location - 11 Concord Street, Haverhill MA  On or about 7/5/2023, a worker without appropriate head protection was struck by falling debris on the head. This individual was positioned at ground level, situated less than 5 feet from a dump truck. At that time, the dump truck was actively being loaded by an excavator, which was transporting debris from the demolition of sidewalks and curbs.
