29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  A).	On or about 07/06/2023, at the site of 157-59 Rose Ave. Flushing, NY  Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and materials from other employees who on the roof throwing them down. Protective helmets were not worn.

On or about 07/06/2023, at the site of 157-59 Rose Ave. Flushing, NY  Employees installing new roof exposed to the hazard of falling approximately 28 ft. to the ground below.
