29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems:  On or about June 29, 2023, an employee working around the Woody Pit located in the Rod Mill Reform Area was exposed to fall hazards of approximately 7 feet 4 inches because the guardrail installed was located approximately 46 inches from the edge of the pit allowing access to the unprotected edge.
