On or about June 15, 2023 two approximately 8-foot tall mobile ladder stands used to access new tires for installation were located next to tire racks measuring approximately 15 feet tall and varying from approximately 2 to 4 feet in depth, exposing employees to a fall hazard by overreaching from the top step.

On or about June 15, 2023 the following instances of flexible cords in use where not approved were observed: a) An energized flexible cord was repaired with adhesive tape. Approval of flexible cord is based on the original condition of the cord; the adhesive tape precluded further examination of the cord's condition. b) A flexible cord without plugs on either end originated from a Cutler-Hammer 30 amp/ 240 volt disconnect switch and terminated in a Worldwide Electric 5 HP motor starter enclosure.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet: Location: Walking-working surface between Northwest service bays. On or about June 15, 2023 an unsecured oxygen cylinder was stored in contact with two unsecured acetylene cylinders. The cylinders were located between two repair bays and exposed to vehicle and foot traffic from the interior and exterior of the building. The oxygen cylinder was also not equipped with a valve protection cap. Pep Boys was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1382020, citation number 1, item number 1 and was affirmed as a final order on October 30, 2020, with respect to a workplace located at 2301 Dixwell Avenue Hamden, CT 06518.
