29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about July 7, 2023, the employees were exposed to a fall hazard when accessing the roof from three different Werner ladders, whose side rails did not extend at least three feet above the landing surface: a. Northeast side ladder, used by all three employees during the inspection. b. Northwest side ladder, used by all three employees during the inspection. c. Southwest side ladder, used by all three employees during the inspection.
