29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about July 7, 2023, employees were exposed to a fall hazard of more than 6 feet while working on the roof without a fall protection system.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about July 7, 2023, the employees were exposed to a fall hazard when accessing the roof from three different Werner ladders, whose side rails did not extend at least three feet above the landing surface:  a. Northeast side ladder, used by all three employees during the inspection.  b. Northwest side ladder, used by all three employees during the inspection.  c. Southwest side ladder, used by all three employees during the inspection.

29 CFR  1926.1053(b)(2): Ladders were not maintained free of oil, grease, and other slipping hazards.   On or about July 7, 2023, employees were exposed to a slip and fall hazard while utilizing a Werner ladder after a rain storm, with slippery mud on the aluminum ladder rungs to ascend and descend from the roof.
