On a 2:12 pitched roof of a residential worksite, located at 372 61st Street, Jacksonville, FL 32208: On or about July 10, 2023, the employer exposed his production manager and 4 subcontracted employees to fall hazards, in that, fall protection was not utilized while tarping a roof, the employer failed to train employees that fall protection is required for that activity.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. At a temporary residential work site, located at 372 61st Street, Jacksonville, FL 32208, : On or about July 10, 2023, the employer exposed his production manager and four subcontracted employees to fall hazards, in that the ladder's side rails were not extended 3 feet above the roof and tied off. The employees were being exposed to fall hazards up to 10 feet.
