On or about July 10, 2023 the employer Josue Caballero DBA Joshua Construction at jobsite located at 6019 Milan Rd. Sandusky OH 44870, did not ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. Three employees installing new roofing plywood and rubber roofing materials to the structure were exposed to falls of 19.9 feet and approximately 11 feet to the ground below.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 10, 2023 the employer Josue Caballero  DBA Joshua Construction at jobsite located at 6019 Milan Rd. Sandusky OH 44870, did not ensure the side rails of two portable Werner brand aluminum extension ladders, and one unknown brand multipurpose ladder, were extended at least three feet above the landing surface. Three employees were exposed to falls from approximately 19 feet and or greater from the top of the second (roof) level, and 11 feet or greater from the building awning to the ground below while using the ladders to access the third (roof) level and second level awning to perform commercial roofing activities on a new build commercial building.
