(a) Worksite - Side of the building: On or about 07/11/2023, the employer allowed employees to assemble and use a pump jack scaffold system with 4 upright poles constructed from two-by-four lumber that was spliced together without mending plates installed at all splices, thus exposing employees to scaffold collapse and fall hazards.
