29 CFR  1910.212(a)(3)(ii): Points of operation on machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycle:   (a)Amcon Concrete Products, LLC located at 101 Industrial Road, Harrisburg, SD 57032 :  On or about and at times prior to July 10, 2023, the employer did not ensure the Tiger Splitter Machine:  model: TSP-12, serial number:  238, in the manufacturing area was properly guarded.  This condition exposed employees to caught-in, crushing and amputation hazards associated with the point of operation.

29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:   (a)Amcon Concrete Products, LLC located at 101 Industrial Road, Harrisburg, SD 57032 :  On or about and at times prior to July 12, 2023, the employer did not ensure the feed mechanism chain and sprocket to the Tiger Splitter Machine:  model: TSP-12, serial number:  238, in the manufacturing area was properly guarded.  This condition exposed employees to caught-in, crushing and amputation hazards associated with the point of operation.
