29 CFR  1910.67(c)(2)(i): Lift controls shall be tested each day prior to use to determine that such controls are in safe working condition.  At the residential address located at 201 East Tripp Street in Mansfield, MO 65708 on or about July 7, 2023: Employees performing telecommunications work were exposed to falls as the lift controls were not being tested. The controls failed to allow the boom to move side to side. The lift controls were not tested each day prior to use. the ground control had been damaged and was not operational.

29 CFR 1910.67(c)(2)(v): A personal fall arrest of travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  At the residential address located at 201 East Tripp Street in Mansfield MO 65708 on or about July 7, 2023: Employees performing telecommunications work were exposed to falls of approximately 20 feet above the ground while working inside the bucket of a lift device without wearing any means of fall protection or a personal fall arrest system.

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not de-energized and grounded, nor were other protective measures provided before work was started:  At the residential address located at 201 East Tripp Street in Mansfield MO 65708 on or about July 7, 2023: Employees performing telecommunications work were exposed to burns, shocks, and electrocutions when working near energized overhead power lines. The employee was working approximately 15 to 20 feet above the ground, inside the bucket of a lift, below uncovered energized power lines. The employee was manipulating a loose copper wire. Contact or arcing between the energized line and copper wire resulted in burns and electric shock, fatality injuring the employee.
