29 CFR  1928.57(a)(8)(iii): Guards shall be free from burrs, sharp edges, and sharp corners, and shall be securely fastened to the equipment or building:  Employees engaged in corn flaking activities at the PMS Roller Steam Corn Flaker machine in the Feed Mill building were exposed to amputation, crushing, and caught-in hazards in that the guards were not securely fastened to the equipment.

29 CFR  1928.57(b)(2)(ii): Revolving shafts, including projections such as bolts, keys, or set screws, except smooth shaft ends protruding less than one-half the outside diameter of the shaft and its locking means, were not guarded:  Employees engaged in corn flaking activities at the PMS Roller Steam Corn Flaker machine in the Feed Mill building were exposed to caught-in and amputation hazards in that the rotating shaft ends on the following were not filed or otherwise covered to protect employees from rotating shaft ends:  a) Flaker North #1 - 1.5 inches diameter, 2 inches long; b) Flaker South #2 - 1.5 inches diameter, 2 inches long.
