Location: Worksite  On or about July 13, 2023, employees were exposed to fall hazards greater than 20 feet from a rooftop to the ground below, where untrained employees were unable to recognize or avoid the hazards, or to wear and use personal fall arrest systems. The employees were not trained on ladders, fall hazards, fall protection, or PPE, and did not receive any other safety or health training from the employer.

The employer was advised verbally and given a written document request, but to this date still has not provided any information.
