29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) At the jobsite located at 14100-14122 Kensington Ln, Fort Myers, FL 33912; on 2/15/23, the employer exposed employees to fall hazards, in that fall protection was not utilized by employees while performing roofing activities such as but not limited to shingle replacement on a 6:12 pitch residential home roof approximately 10 feet in height.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) At the jobsite located at 14100-14122, Fort Myers, FL 33912; on or about 2/15/23, the employer exposed employees to fall hazards, in that the ladders were not 3 feet above the upper surface when utilized by employees while accessing the roof of a residential home to perform work activities.
