29 CFR  1910.1053(f)(2)(i): The employer did not establish and implement a written silica exposure control plan:  At or near 2405 Z St. - Omaha, NE 68107 - Employer failed to establish and implement a written respirable silica exposure control plan that contained the required elements of this section. The program and training records were not available upon agency request.  Employees are exposed to respirable silica dust from the machine handling and crushing of recycled concrete. This hazard was last observed on or about November 22, 2023.

29 CFR  1910.1053(j)(1):The employer did not ensure that each employee was trained in accordance with the provisions of the Hazard Communication Standard and paragraph (j)(3) of this section:  At or near 2405 Z St. - Omaha, NE 68107 - Employer failed to ensure that each employee received the appropriate silica hazard training required by the silica and hazard communication standards. Employees are exposed to respirable silica dust from the machine handling and crushing of recycled concrete. This hazard was last observed on or about November 22, 2023.
