29 CFR  1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section:   On or about 7/14/2023, at 11929 Southern Blvd, Royal Palm Beach, Florida, an employee was exposed to an approximate 20-foot fall hazard while working on open roof trusses without fall protection.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about 7/14/2023, at 11929 Southern Blvd, Royal Palm Beach, Florida, an employee was exposed to a fall hazard while using a ladder that did not extend at least 3 feet above the upper landing surface.
