29 CFR  1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided:  On or about July 18, 2023, at a jobsite located at the intersection of 11th Street and Broadway Avenue in Sheboygan, WI, employees entering and exiting the trench box were stepping across a 2 to 3 foot opening between the top of the trench box edge to the trench soil edge where the trench box built-in ladder and grab rails were located.  Dorner, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1051(a), which was contained in OSHA inspection number 1631926 citation number 1, item number 1b and was affirmed as a final order on December 7, 2022, with respect to a workplace located at North Orion Lane and West Navitus Drive in Grand Chute, WI.
