29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is four (4) feet or more above a lower level was protected from falling: a) Docking Bay 5 - On or about July 19, 2023, employees manufacturing paper bags were exposed to a fall hazard while working on a walking-working surface with an unprotected edge that was approximately four (4) feet one (1) inch from the lower level. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Manufacturing Room: On or about July 19, 2023, the employer did not develop an energy control program consisting of written machine specific procedures, periodic inspections, and employee training for both authorized and affected employees exposed to crushing hazards while performing servicing and maintenance activities such as but not limited to clearing paper jams in calender rollers. ABATEMENT CERTIFICATION REQUIRED
