29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  a) Throughout the facility - On or about July 20, 2023, and at times prior to, employees were exposed to crushed by hazards in that, the employer failed to ensure that storage of stone slabs were secured against sliding and collapsing when employees were handling and moving the slabs of granites in the warehouse.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a) Throughout the facility - On or about July 20, 2023, and at times prior to, employees were exposed to struck by hazards in that, the employer failed to ensure that an evaluation of powered industrial truck operator's performance was provided to powered industrial truck operators at least once every three years while employees were operating PIVs such as Yale and Hyster forklifts.
