Locations:  a) A doorway entry into the lower level of the project where there was rebar bent over and other debris was at ground level. b) Construction and other debris in the lower level of the project. c) Construction and other debris around the exterior of the project.  On or about July 24, 2023, employees were exposed to slip, trip, and fall hazards where bent rebar and debris were not cleared from an entryway, or from in and around the building.

Locations: a) Unguarded windows on 2nd level of project, on side and back of building:  On or about July 24, 2023, employees were exposed to falls of an estimated 20 feet where windows were not guarded and the inside bottom edge of the window was at 20 inches.  b) Unguarded windows on 2nd level, back wall of material hoist area: On or about July 24, 2023, employees were exposed to falls of an estimated 8 feet where a window was not guarded.

Location: Worksite  On or about July 24, 2023, employees were exposed to fall hazards of up to 30 feet from the rooftop to the ground below, where untrained employees were unable to recognize or avoid the hazards, or to wear and use personal fall arrest systems. The employees were not trained on ladders, fall hazards, fall protection, scaffolds, or PPE, and did not receive any other safety or health training from the employer.

Locations:  a) Rebar stubs located on the lower level of the project at the edge of an unprotected excavation.  b) Uncapped rebar located on the 2nd level of the project.  On or about July 24, employees were exposed to impalement hazards where rebar was not capped, as well as slip, trip, and fall hazards.
