On or about July 25, 2023, the employer failed to provide a medical evaluation to employees required to wear half-face respirators with cartridges while drilling holes in the tanks.

29 CFR  1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT).  On or about July 25, 2023, the employer failed to ensure employees required to wear a tight-fitting half-face facepiece respirator with cartridges while drilling holes in the tanks were fit tested prior to initial use of the respirator.
