On or about July 26, 2023, employees were exposed to slip, trip, and fall hazards in that:  a) Water was accumulated on the floor from a roof leak in the break room area.   b) Water was accumulated on the floor from a roof leak in the warehouse.  c) Items such as, but not limited to a pallet, tubing, and trash was located behind the freezer.  d) Water was accumulated on the floor inside refrigerator.  e) Ice was located on the freezer floor.
