29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s).  a) Silo area: On or about July 26, 2023, and at times prior,  employees are exposed to struck by hazards in that the employer allows defective forklift to operate. A forklift with reported deficiencies was used to lift a 2200lb bag of garnet 10 feet in the air to fill a silo. While the operator was positioning the bag over the silo, the forklift shifted and slipped off the forks.
