29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  Jobsite,  760 Cooper Ave, Johnstown, PA 15906: Observed on or about July 27, 2023, employees engaged in roofing activities on a roof with a slope of 4 in 12 (vertical to horizontal) were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  The employees were exposed to a fall hazard of up to approximately 13 feet to the ground below.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  Jobsite,  760 Cooper Ave, Johnstown, PA 15906: Observed on or about July 27, 2023, employees engaged in roofing activities were using a 32 feet/300 pound capacity Werner brand portable extension ladder to access the roof.  The side rails of the ladder did not extend at least 3 feet above the upper landing surface.   Employees using the ladder were exposed to a fall hazard of approximately 13 feet to the ground below.
