29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  Jobsite,  760 Cooper Ave, Johnstown, PA 15906: Observed on or about July 27, 2023, employees engaged in roofing activities were using a 32 feet/300 pound capacity Werner brand portable extension ladder to access the roof.  The side rails of the ladder did not extend at least 3 feet above the upper landing surface.   Employees using the ladder were exposed to a fall hazard of approximately 13 feet to the ground below.
