29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary:  a. At 3176 Old Jennings Road, Middleburg, FL; On or about July 27, 2023, employees of Aquino Construction, Inc. were working in a 8 foot trench, where the spoil piles were placed on the leading edge of the trench, exposing employees to being struck by the material.

At 3176 Old Jennings Road, Middleburg, FL; On or about July 27, 2023, employees of Aquino Construction, Inc., in a 8 foot deep trench, were not protected from cave-in from walls by the use of a protective system.
