29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.   a- 691 Wyckoff Ave, Mahwah NJ 07430 (Residential Roof): CSHO observed employees engaged in roofing activities using Hitachi Pneumatic nail guns were exposed to eye loss and eye injury as a means of eye protection was not being utilized. Observed on or about 07/26/2023.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)(1)  a- 691 Wyckoff Ave, Mahwah NJ 07430 (Residential Roof): CSHO observed employees engaged in roofing activities on a 8 on 12 and 16 on 12 pitch roof in which a means of fall protection was not being utilized exposing the employees to fall hazards of approximately 15 & 25ft to the asphalt below. Observed on or about 07/26/2023.
