29 CFR  1910.28(b)(8): Where there were repair pits, service pits, and assembly pits less than 10 feet in depth, the employer did not limit access within 6 feet (1.8 m) of the edge of the pit to authorized employees trained in accordance with � 1910.30; place floor markings at least 6 feet (1.8 m) from the edge of the pit in colors that contrast with the surrounding area; or provide a warning line at least 6 feet (1.8 m) from the edge of the pit with stanchions capable of resisting, without tipping over, a force of at least 16 pounds (71 N) applied horizontally against the stanchion at a height of 30 inches (76 cm); provide a combination of floor markings and warning lines at least 6 feet (1.8 m) from the edge of the pit, or posts readily visible caution signs that meet the requirements of �1910.145 and state ""Caution-Open Pit.":  a) Pioneer Quick Lube Napoleon 07, on or about July 28, 2023, at the address of 1108 Scott St. Napoleon, OH: Employees servicing vehicles in a workplace that contained two (3ft.x16ft) pits without fall protection systems in place were exposed to falls of 5'9" to the concrete level below. The employer failed to utilize measures to include training authorized employees on fall hazards, applying contrasting floor markings at least 6ft. from the edge of pit(s), warning lines and stanchions, or a combination of lines and floor markings and posting readily visible caution signs that state "caution-Open Pit."
