29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet:  On or about July 28, 2023 and at times prior thereto, employees working inside an excavation at a depth over four feet did not have a ladder for safe access and egress from excavation, exposing employees to caught-in hazards.

29 CFR  1926.652(b)(3)(i): The designs of sloping or benching systems were not selected from and in accordance with tabulated data, such as tables and charts.   On or about July 28, 2023 and at time prior thereto, employees were exposed to a caught-in hazard while replacing sewer lines in an excavation greater than 5 feet with inadequate sloping and benching.
