29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about July 28, 2023, employees were exposed to a struck-by hazard while using a pneumatic air nail gun to install shingles on a roof without proper eye protection.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 28, 2023, employees were exposed a fall hazard of greater than 6 feet to the ground below, while attaching roof shingles, without being provided effective training in fall hazards.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  On or about July 28, 2023, employees were exposed to a fall hazard while accessing the roof decking using a ladder where the ladder side rails did not extend at least 3 feet above the upper landing surface.
