29 CFR  1910.272(j)(2)(ii):The employer did not immediately remove  fugitive grain dust accumulations whenever the exceeded 1/8 inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators:  In the grain elevator, employees are exposed to burns and injuries associated with combustible dust. Grain dust had accumulated to greater than 3/4 inches near bin  covers 21 and 23 beneath the conveyor belt at the top level of the grain elevator.

29 CFR  1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  In the grain elevator on the top floor near the man lift, employees were exposed to slip, trip, and fall  hazards from screws protruding more than 1.5 inches from a metal floor plate.
