(a) On or about July 13, 2023, employees servicing ATM machines were not wearing body armor (bullet proof vest) which exposed employees to gunshot wounds. The employer did not assess each workplace to determine when body armor was needed.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: (a) On or about July 13, 2023, employees servicing ATM machines were not wearing body armor (bullet proof vest) which exposed employees to gunshot wounds. The employer did not provide each employee with the proper personal protective equipment (bullet proof vest) during the first 90 days of employment, and did not ensure each affected employee used the proper personal protective equipment (bullet proof vest) when servicing ATM machines.
