On or about: July 31, 2023 Location: 210 North Broadway, Sleepy Hollow, NY 10591  a) CSHO observed one-tier tubular welded frame scaffold on site not equipped with base plate, mud sills or other adequate firm foundation which was used by the injured worker.

On or about: July 31, 2023 Location: 210 North Broadway, Sleepy Hollow, NY 10591  a) CSHO observed one-tier tubular welded frame scaffold on site with cross-bracing not secured into place. The locking pins were not in an upright position which would secure the cross braces into place.
